We provide all the necessary forms needed for closing packages.
We will forward the following forms with each pre-treat:
- New Construction Subterranean Termite Soil Treatment Record (Form NPCA-99b)
- Subterranean Termite Treatment Builders Certification and Guarantee (Form NPCA-99a)
- Termite Service Agreement - In a bound folder
- Invoice for work
The only form that needs to be returned back to us is the white copy (the first signed page) of the Termite Customer Agreement from the new homeowner.
